My Story
I’m Jennifer “Jen” Allen, a Credentialed Diabetes Educator (CDE) with the Australian Diabetes Educators Association (ADEA) and a Registered Nurse based in Port Macquarie, NSW. I am passionate about supporting people living with diabetes to better understand their condition, build confidence in self-management, and live a healthier, more fulfilling life.
I provide care for children, adolescents, and adults with Type 1, Type 2, gestational and steroid-induced diabetes. My background includes caring for children with both acute and chronic conditions in hospital settings, as well as guiding adolescents and adults in community and private practice.
Over the years, I have worked in leading hospitals and health services, including St Vincent’s Hospital Chermside, Queensland Children’s Hospital, and Queensland Children’s Health, alongside private practice in Brisbane. This experience allows me to offer practical, evidence-based support that is always tailored to the individual and their unique circumstances.
At Evolve Diabetes, my focus is on providing education, strategies, and encouragement to empower you to manage your diabetes with confidence — whether you are newly diagnosed or have lived with diabetes for many years. Together, we will create a plan that works for you and helps you thrive in everyday life.

Referrals
Evolve Diabetes accepts referrals from General Practitioners, Specialists, and Allied Health Professionals. Referrals can be made via secure messaging through Medical Objects, email or letter and should include:

Patient demographics and relevant medical history

Current medications and diabetes treatment

Specific referral reason or education goals

Details of a GP Chronic Condition Management Plan (if applicable)
Patients referred under a GP Chronic Condition Management Plan may be eligible for a Medicare rebate for diabetes education services. Following assessment, Evolve Diabetes provides feedback to the referrer regarding progress and ongoing management needs, ensuring collaborative, patient-centred care.
